Joseph White Terms & Conditions Dog Obedience Waiver, Assumption Of Risk And Agreement To Hold Harmless.

Services are non-transferable to other dogs or people and must be used within six months of form submission.
There will be no refund/credit/prorate for cancellation or termination of service. Sessions are lost without 24 hour prior notice for re-scheduling. Sessions/classes are for a duration of 45 minutes and start exactly at the scheduled times. If you are not present at the time scheduled it will be assumed your intention is to forfeit the appointment ( I typically wait for 3-7 minutes in hope the time will not be a total loss for the client).
Anyone under 18 years of age must have parent or guardian submit form.

I understand that attendance of a dog obedience training class, seminar or private training is not without risk to myself, members of my family or guests who may attend, or my dog, because some of the dogs which I (we) will be exposed, including my own, may be difficult to control and may be the cause of injury even when handled with the greatest amount of care.

I here by waive and release Joseph White, Animal Care Services, it’s employees, owners and agents from any and all liability of any nature, for injury or damage which I or my dog may suffer, including specifically, but not without limitation, any injury or damage resulting from the action of any dog, and I expressly assume the risk of any such damage or injury while attending any training session or other function of Joseph White or Animal Care Services, or while on the training grounds or the surrounding area thereto.

In consideration of and as inducement to the acceptance of my application for training membership in this obedience training class, seminar or private training, I hereby agree to indemnify and hold harmless Joseph White, Animal Care Services, its employees, owners and agents from any and all claims, or claims by any member of my family or any other person accompanying me to any training session or function of Joseph White, Animal Care Services or while on the grounds or the surrounding area thereto as a result of any action by any dog, including my own.

I also acknowledge responsibility of any equipment loaned to me by Joseph White, Animal Care Services, it’s employees, owners and agents. This responsibility is equal to a new business agreement between myself & Joseph White, Animal Care Services, it’s employees, owners and agents. This agreement gives permission to Joseph White, Animal Care Services, it’s employees, owners and agent to use any payment form/ information already acquired for other agreements to replace at full retail cost any equipment damaged in my care, non-returned equipment or late returned equipment. I also, assume all responsibilities, burden and effort involved in the return of equipment.

Class attendance or sessions might be withheld, without refund, based on Joseph White's sole decision if he feels it is determined potentially detrimental to continue.
